Comprehensive Primary Care Plus is a national advanced primary care model intended to improve primary care efficiency, quality, and access through multi-payer payment reform and care delivery transformation. It’s a public-private partnership in which payers in 18 regions support health care practices. Comprehensive Primary Care Plus includes 2 primary care practice tracks with different advanced care delivery requirements and payment options.
